Has anyone yet mentioned that income and obesity are inversely correlated in the US? One can think of many reasons for such a relationship, but my point would be this - it will be one more confounding variable, since (I suspect) high earners are less likely to die (within, e.g, a one year time horizon) than others in the same age cohort.
