I haven't played San Andreas, but if that's anything like beating all 12 levels of Ambulance missions on GTA3, I recommend it for the sense of personal accomplishment.
I got through it twice without having done the firefighter missions, which are a lot harder to complete (in my opinion) than it is to finish End of the Line without being fireproof.
