Does camp have to be self-aware? Like, we know what Susan Sontag would think of King Diamond (awesome), but what about Korn? You can't get less camp than Korn, can you?

Most (good) metal has a sense of consciously exaggerating for effect, but I wouldn't call it "camp," really. Take Metallica's "Sanitarium" -- the lines "kill is such a friendly word, seems it's the only way, of reaching out again" is supposed to be kind of funny butisn't really making fun of itself, it's also supposed to sort-of-seriously reflect the despair and channeled rage of the metal fan base.
Metal is about nerdy, angsty teens (mostly guys) feeling powerful by rocking out." That's all.

WOW! All I could think was, "New! Shimmer! It's a floor wax, it's a desert topping!" Heavy metal AND disco! the two most taste-free genres I know, together! I couldn't wait to push the "stop making it hurt" button!
